Preparing For Your Appointment

Thank you for scheduling a consultation at The Law Collaborative. Please download and print the client intake forms below, fill them out prior to your appointment, and be sure to bring them with you when you come in. You do not need to print all the forms - only those which apply to your case. If you are not able to fill them out in advance, please be sure to arrive for your appointment fifteen to twenty minutes early, so that you have plenty of time to fill them out at our office before your appointment begins.

Your initial consultation is designed to fully explore your legal concerns and to provide you with good advice. Our family law attorneys will address your concerns, answer your questions, and design a strategy that works best for your unique case. Please be advised that to fully assess your case and determine your needs, our attorneys require a full and thorough interview with you. In order to better assess your case, it is recommended that you bring the following items for attorney review:

• All deeds of all properties owned
• Tax assessments on each property
• IF you own a house, then the latest monthly statement of payment, principal, and interest
• IF not already divided, an itemization of all household furniture and furnishings worth more than $200.00, list room by room and note those items that were yours prior to marriage or that were inherited during marriage
• List any special “collectibles” i.e. dolls, coins, gold, stamps, guns, sporting cards, and other collections worth more than $500.00
• Tax returns for the past 3-5 years
• Copy of each monthly debt including credit cards, car payments, auto/health-medical/life insurance premium payments, and all utility bills
• Copies of all bank account statements for past three months (if date of separation is not current date), then three months before and after that date
• Any apartment rental agreement
• All life insurance policies
• Last three paychecks or last paycheck evidencing year to date income
• License numbers, pink slips on all vehicles, i.e. cars, boats, motorcycles, jet skis, etc.
• IF you have pensions, stock plans, or 401K’s, bring copies of your last statement
• Any legal documents or agreements pertaining to your current legal proceedings
